COVID-19 PREVENTION: NEW PROCEDURES AND PRECAUTIONS

1. BEFORE YOUR INITIAL VISIT
• All new patients have to download and fill out Confidential clinic intake form. The form has to be sent us by email attachment 48 hours prior to the scheduled appointment.

—No patients are allowed in the office if you are currently exhibiting any symptoms of COVID-19 (or if you have any time in the past 10 days).
These symptoms include:

・Fever ・Dry Cough・Shortness of breath ・Shaking chills ・Loss of taste or smell・Sore throat

Also, please answer the following questions:
• Have you had close contact with or cared for someone diagnosed with COVID-19 within the last 7days?
—If you answer is “Yes” to any of above questions, please contact us to cancel / reschedule.
*There are no late cancellation fees for illness, so please cancel/reschedule your appointment if you are not feeling well.

2. DURING YOUR TREATMENT
• PPE will be worn by practitioners, which include gloves & masks. All practitioners in the space will be screened for fever and any new symptoms at the beginning of each shift.
• We will use disposal plastic table covers for each new patients.
You will be provided with a personal “Thermal Blanket”, which you can take home if you prefer.
